[made under section 50 of the Taxes Management Act 1976 [title 14 item 40] and brought into operation on 8 May 1976]

1. Forms

(1) The Forms set out in the Schedule shall be used for the purposes of the Taxes Acts with such variations as the circumstances of the case may require.

(2) The following are the forms set out in the Schedule—

(a) Form L1, being the form of application for registration under section 6 of the Taxes Management Act 1976 [title

14 item 40] by a person chargeable to hospital levy;

(b) Form E3, being the form of return to be furnished for the purposes of section 7 of the Taxes Management Act 1976 in respect of employment tax and hospital levy;

(c) Form B1, being the form of return to be furnished for the purposes of section 7 of the Taxes Management Act 1976 in respect of betting duty;

(d) Form B2, being the form of the report of an approved auditor which is required by section 33 (1) (b) of the

1989 Revision 1 TAXES MANAGEMENT (PRESCRIBED FORMS) REGULATIONS

Miscellaneous Taxes Act 1976 [title 14 item 46] to accompany Form B1.
